Registration Form for Students of BS-UHE
Instructions to Fill the form
Before submission makes sure that:
- Print the form on an A4 paper.
- Fill with Capital Letters.
- Make sure all the spellings are correct.
- Attach an attested copy of Matriculation/O & A level results.
- CNIC of the student/ B form certificate copy.
- Fathers’ CNIC copy.
- Passport Size photograph
- All sections are to be filled except the ones mentioned. Incomplete forms will not be accepted.
- You are requested to submit the registration form to your respective Department.
